Custom orders take a minimum of 1–2 weeks to process. After you send this form I’ll confirm details, extras, and how to send a photo if you need one.

How to fill this in
- 1. Hail Mary beads Choose the colour bead you would like for the Hail Mary prayer — 10 beads in each decade.
- 2. Our Father beads Choose a different colour bead or accent (caps, halos, and the like) for the Our Father prayer — 6 beads in the rosary.
- 3. Metal Decide on gold or silver for the cross and pendant.
- 4. The personal part A name, date, initial, or photograph can be added. For pins, list the numbered charms you want.
For pins, the number of charms you request should match the pin you selected. Charms 41 and up are +$5 each. A second hanging row of beads is +$10. A custom photo pendant on a pin is +$30.
